With the repeater input on 1249MHz and the output as high as 1316MHz, is it feasible to use just one antenna switched between TX and RX? Or are two antennas needed, tuned to the different frequencies?
Or do people usually work crossband - 70cm in, 23cm out?

The perfect aerial for 23cm repeater working was the old G3JVL quad-loop, which had three driven elements on a log periodic feed and covered the frequency split well. The design is around if you want to have a go at making one.
I am currently using the smallest Wimo beam;

This covers 1249/1316 quite well and is a very well made aerial that has been up for over 15 years and is still working well. Ordering direct from Wimo in Germany is no problem and orders arrive within a week.
